Care Forward is the service delivery arm of Care Assessment Consultants Pty Ltd, a locally owned and operated Tasmanian community and healthcare service provider. We’ve been helping people across this State live well at home since 1999. Our service coordinators are all Registered Nurses or an equivalent Allied Health Professional. This means you have the security of knowing your service coordinators have the clinical expertise to provide you with the highest quality of support to enhance your independence, health and wellbeing. We provide specialised independence programmes with a focus on strength, balance, wellness and confidence, to work with clients to regain lost abilities and/or learn new skills to remain living at home independently.
Care Forward can provide any combination of government subsidised or fee-for-service including but not limited to: Commonwealth and Tasmanian Home Support programs for individuals who are finding things a bit difficult around the home and would like to work with health professionals to maintain independence or need short-term assistance to remain at home; Home Care Packages for eligible ACAT (Aged Care Assessment Team) assessed clients, with consumer directed care planning for all levels of care; Allied health services including exercise physiology, occupational therapy, dietitian, and social work consultations; Group exercise classes; Individual strength, mobility, balance and activity programs; Private programs for individuals or organisations able to pay a fee-for-service.
